Skilled Nursing Facility in Massachusetts

Sale of substantially all of the real estate assets owned by the seller and located at 20 Kinmonth Road, Newton, Massachusetts 02468 (the "Property"). The Property is leased by Seller to Waban Health Center, LLC ("Operator") that uses the Property as a skilled nursing facility (the "Facility").

The real estate assets includes: (i) the representative Property consisting of that certain plot, piece or parcel of land located in Newton, Massachusetts, as more particularly described in Exhibit A on the attached PDF (the "Land"); (ii) all buildings and all other structures, facilities or improvements presently or hereafter located in or on the Land utilized by the Facility (the 'improvements"), (iii) all other fixtures attached or appurtenant to, located on or used in connection with the ownership, use, operation or maintenance of the Land and the Improvements (collectively, the "Fixtures"); and (iv) the land lying in the bed of any street or highway in front of or adjoining the Land to the center line thereof, if any.
